8/30/2023 EnchantaVaughn Approved Behavior: The home was neat and in order when QMHP arrived. It appeared that client finished all of his chores and was ready to complete his therapy. Client remained polite and patient as his counselor gathered things for the session and psycho educational activity. Behavior: Client asked to lead the next activity and when doing so he spoke with confidence and high self-esteem. Client is observed displaying age appropriate manners, avoiding distracting behaviors while stating to QMHP-C "i get angry because my sister is always picking on me". Intervention: QMHP-C engaged mother in a discussion to determine behaviors since last session.QMHP provided and demonstrated empathy, compassion, and support to help him become more comfortable in the session. QMHP provided strategies such as positive reframing. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations. QMHP inquired with client his feelings, moods, and behaviors. QMHP reviewed and discussed objectives and goals that will allow and teach client how to channel his reported behaviors within the home, school, and community environments. Intervention: QMHP led the family through each treatment exercise. QMHP engaged the client in a role reversal exercise and required the client to think about his goals for the future. QMHP utilized positive self-talk strategies to help client think positively at all times. QMHP taught client effective problem-solving skills (i.e. identifying the problem, brainstorm alternate solutions, select an option, implement a course of action, and evaluate). QMHP encouraged mother to allow client to express himself in an age appropriate manner and to redirect when needed. Response: Mother reports client struggles with expressing his feelings without being angry. Client was assisted in practicing general communications skills. Client understood effective problem-solving skills to help solve or overcome a problem or stressor the he is currently facing. Client demonstrated good calming skills by deep breathing and having good voice volume. Client recognized when heâs not paying attention and quickly corrected himself. Client understood that when he becomes agitated he needs to refrain from taking it out on people and to communicate how he is feeling to others so that everyone is on the same page. Response: Client was receptive and open to the interventions, as he was attentive and focused on the information and activities that were being processed without any difficulties. Client displays good social behavior, as observed and supported by mental health professional. Client and mom were observed interacting well with one another. Client is reported building healthy relationships outside of the home as well. Client utilized meditation and relaxation techniques. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques
